Some people get paralyzed by fear of "what might have happened but didn't". They keep reliving the narrow escape, becoming less alert, and putting their charges in even more danger.
I can't speak for everyone, but I focus on adjusting procedures to prevent future near misses of the same kind. It's a good thing kids grow up, otherwise the rules and procedure would start to resemble the Talmud.
